Integrated Team Care (ITC)
Moorundi Aboriginal Community Controlled Health Service Ltd. delivers a comprehensive range of primary health care services to our communities.
The ITC program is designed to provide you with support to better manage your chronic condition and lead a healthier life.​

Our team aims to contribute to improved health outcomes for Aboriginal and Torres Strait Islander people living with a chronic health condition through better access to culturally appropriate care.
Eligible Patients

Identify as an Aborignal and/or Torress Strait Islander person.

Have a chronic condition that has been present for at least 6 months, including but not limited to diabetes, cancer, cardiovascular disease, chronic respiratory disease, chronic kidney disease, eye health conditions associated with diabetes, and mental health conditions.

Have a current GP Management Plan.

Living on Ngarrindjeri Ruwe, our service area incorporates the Adelaide Hills, Fleurieu, Murray Mallee ( Mallee/Coorong ) and Kangaroo Island.
